Participation in Brazilian Jiu-Jitsu, striking, auxiliary training, camps, seminars, competitions, observation, or related activities at Ares Peoria requires a signed liability waiver / release before you step on the mats.
Usually: finish your paperwork during check-in—we’ll walk you through it a few minutes before your first class.

- Bring a photo ID where appropriate and list any minors on the parental/guardian section your form requires.
